本学におけるクリニカル・クラークシップ / Clinical Clerkship

The clinical clerkship at St. Marianna University School of Medicine, which lasted about two months, gave me a lot of beautiful memories and learning experiences. This made me realize that I would like to work at a Japanese hospital in the future.
Due to the COVID-19 pandemic, all the classes in my university at Peru were switched to online. Although I preferred face to face classes, the practices for clinical clerkship were also conducted all online. Even at difficult times during the COVID-19 crisis, fortunately, St Marianna University School of Medicine kindly accepted me for the clinical clerkship during my summer vacation. I was able to experience seven departments and I learned as much as I could from each specialty.
From the first day, the office staff kindly showed me around the facilities of the university and hospital. If I encountered any problems, they helped me immediately so I was able to proceed smoothly with the clerkship until the last day.
I attended conferences with the doctors every day. Additionally, after making rounds, I was able to see patients in the ward, outpatients, and even observe surgeries. Throughout my experiences, I was impressed by the Japanese cleanliness management, technology, discipline, observance of rules, and professionalism. I would like to introduce and adopt what I have seen to the hospitals in Peru.
When I participated in the research presentation by a fourth grade student and heard their excellent work, I was impressed by the level they managed and the wonderfulness of the contents. Like the presenter, I thought that I should be more enthusiastic about research. Now that I am back in Peru, I am looking for a research theme in obstetrics and gynecology which is my area of greatest interest.
When the clinical clerkship for the fourth grade students of this university started, I was able to learn with them. Everyone was kind to me, and I had lunch with them and we taught our study technics to each other. I had a very fulfilling, insightful, and motivating time with them because I am planning to take the Japanese national examination in the future.
Even now, I still remember clearly when I first assisted a Caesarean surgery. Moreover, I cannot forget the moment when a baby was born safely and cried. After that, my teacher taught me the procedure step by step, and I was able to learned more. What I received with my five senses when I entered the operation room is a precious experience that I will cherish throughout my lifetime. Furthermore, I was able to be in a OR of a kidney transplant, touched polycystic kidney with my own hands, and another OR of a partial resection of liver cancer for 9 hours straight without drinking and eating. Everything was new and exciting for me.
I would like to express my deepest gratitude to all the doctors at St. Marianna university hospital, including Professor Naoko Suzuki, for their generosity, kindness, guidance, expertise, and patience.
Taking this experience, I would like to study hard every day so that I can become a bridge between Peru and Japan's medical care in the future.

およそ2か月間にわたる、聖マリアンナ医科大学病院でのクリニカルクラークシップでは、たくさんの思い出と経験を得ることができ、将来日本の病院で働きたいと思いました。
コロナ禍において、自分の国ペルーの大学の授業はすべてオンラインであり、4年生になって病院実習が始まっているはずの時期もすべての実習はパソコンでのみ行われていました。そんな中、大学の夏休み期間に聖マリアンナ医科大学がクリニカルクラークシップを受け入れてくださいました。私は7つの科を回り、学ばせていただきました。
初日から事務の方が親切に大学と病院の施設を案内してくださいました。そして最終日まで困ったことがあった場合にはすぐに手助けしてくださったおかげで、スムーズにクラークシップを進めることができました。
毎日、先生方とともにカンファレンスに参加し、回診をした後、病棟の患者さんを診たり、外来にくる患者さんを診たり、時には手術も見学することもできました。その際、日本の清潔管理、技術、規律正しさ、ルールを守る姿勢、そしてプロフェッショナリズムに感動しました。ペルーの病院に取り入れていきたいです。
同じ4年生の研究発表会に参加し優秀作品を聞いた際、その難易度と内容のすばらしさに感激し、私ももっと研究に対して意欲的になるべきだと考えました。ペルーに帰ってきた今、自分の一番興味がある分野である、産婦人科の研究テーマを探しています。
本大学の4年生の実習が始まり、一緒に回った時はみなさん優しく接してくれて、彼らとお昼ご飯を食べたり、お互いの勉強方法を教え合うことができたりました。将来日本の国家試験受けたいと思っている私にとっては、とても充実した毎日を送ることができました。
今でも、初めて帝王切開の手術に入り赤ちゃんが無事生まれた瞬間を忘れられません。その後、先生が手順を丁寧に教えてくれて、より勉強になりました。そのほかにも、手術室に入り、自分の五感で受け取ったものは一生忘れられない貴重な経験ばかりでした。腎移植の手術に入り、他嚢胞性を自分の手で触り、肝臓がんの部分切除の手術で9時間飲まず食わずになったときなど、すべてが初めての経験で楽しかったです。
鈴木直先生を始め、担当していただいた先生方、丁寧且つ親切に教えてくれて、心から感謝しています。
この経験を生かして、将来、ペルーと日本の医療の架け橋になれるように日々勉学に励みたいと思います。

My five months at St. Marianna University School of Medicine were wonderful. Each week I spent one day at the University and one day at Tama-Kawasaki Municipal Hospital. I had the opportunity to work with MD/PhD students planning research projects in a weekly seminar. I also delivered three lectures on public health. I enjoyed meeting faculty and students and had many productive conversations on Long COVID, challenges to deprescribing medication, long-term care for older adults in Japan, and end-of-life care. I was able to observe medical home visits as well as the Tama Hospital primary care clinic.
My hosts at St. Marianna were Dr. Kenya Ie and Tina Tajimi. I first met Dr. Ie when he was a Fellow in Family Medicine at the University of Pittsburgh. I served as Dr. Ie’s advisor as he completed his training in public health. We have collaborated on a number of projects since then, with many publications. We are currently conducting parallel studies of medication deprescribing in Japan and the US. During my stay we refined the protocol for this study and successfully translated a measure of readiness to deprescribe. I worked with a number of his students who are conducting research on medical education, college student mental health, and alcoholic liver cirrhosis.
St. Marianna staff were wonderful hosts and helped me navigate the challenges of temporary residence in Japan. They helped us with opening a bank account, registering with the local ward office, arranging for COVID booster vaccination, travel and tourism, and even a gym membership. They tolerated my attempt to speak some elementary Japanese.
I look forward to continuing collaboration, a return visit, and a chance to serve as a host in the US for St. Marianna students and faculty.
